Just like all other health professionals in the US, I’m required to provide you with my HIPAA Notice of Privacy Practices. Please open and review it before your first session with me. It’s a separate document from my website privacy statement below.
Welcome. The purpose of DarylAnnSmith.com (“Website,” “we,” “us,” “our”) is to familiarize people with, and connect people to, the Oahu-based professional services of Daryl Ann Smith, RDN, LD, CEDRD, who specializes in nutrition therapy for the treatment of eating disorders.
Our central premise is confidentiality in all interactions with anyone who connects with us via the Website (or any other means).
Unlike many other websites, DarylAnnSmith.com has no ads, no affiliate links, and no newsletter. So, there is a minimal collection of personal and non-personal information required to fulfill the purpose of the Website.
The good news? The Website exists to support your quest for recovery to normal eating, not to snoop on you or to spam you.
Information We Collect About You
When accessing the Website, we will learn certain information about you, both automatically and through voluntary actions you may take, during your visit. This policy applies to information we collect on the Website and in email, text, or other electronic messages between you and the Website.
Personal Identification Information You Provide to the Website. We collect information that Users and Patients provide via email to the Owner; via search queries on the Website search feature; and in connection with other activities, services, features or resources we make available on this Website. We use the information collected to deliver the requested information.
Users may be asked for, as appropriate, name, email address, mailing address, and credit card information (to secure an appointment, for example). Users may, however, visit our Website anonymously. We will collect personal identification information from Users only if they voluntarily submit such information to us. This Website may collect and use Users’ or Patients’ personal information for the following purposes:

Information you provide helps us respond to your service requests and support needs.

We may use information in the aggregate to understand how our Users as a group use the services and resources provided on the Website.
- To share Patients’ experiences
With permission of a particular Patient, we may anonymously post a Patient’s testimonial about our services.

We may use feedback you provide to improve our products and services. In addition, we may use some of the questions sent to us by Users or Patients, or asked of us via telephone or other means, to develop content for the Website. However, in these cases, all personal identifying information is stripped or modified to protect the privacy of the User or Patient.
NOTE: We do not sell, trade, or rent Users or Patients personal identification information to others.

If you choose to correspond with the Owner via email, she may retain the content of your email messages together with your email address and the email responses. Correspondence is protected and kept confidential. As a matter of policy and practice, we do not sell, rent, lease or otherwise transfer any information collected whether automatically or through your voluntary action.
Compliance with Children’s Online Privacy Protection Act
This Website is not intended for children under 13 years of age and no part of the Website is structured to attract anyone under 13. No one under age 13 may provide any information to or on the Website. We do not knowingly collect personal information from children under 13.
